GM case 
Hi I,am  P.Samhitha of  of 3rd BDS . This is an online e-blog to discuss our patient health data after taking her consent . This also reflect my patient centred online learning portfolio.

Chief complaint- patient complaints of having abdomen pain radiation from front to back with constipation from past 1month and no proper digestion with normal appetite

History of present illness -Distended abdomen upper right and left side with pain radiating from front to back with constipation from 1month Pain when food not digested and no bowel movements.Used to relieve with medication but developed severe pain. No vomiting and diarrhea.Pain more during night.

History of past illness -
No hypertension
No diabetes
Vomiting and diarrhea 3mojths back and relived with medication.
Blood transfusion 4 days ago

Family history - no similar history 

Personal history - wakes up early morning and goes to work. Used to skip meals during work 3yrs ago and started pain
Diet is vegetarian and stoped eating non veg 2 years back and stopped drinking alcohol from 2yrs back because of pain.No proper sleep because of pain.No proper stools.Micturation is normal 

General examination -
No pallor
No lymadenopathy
Temperature -normal
Pulse rate -78beats/min
Blood pressure -120/70mm Hg
